BE WELL KIDS FOUNDATION ACCIDENT & LIABILITY WAIVER I, the undersigned parent or legal guardian of the above-named participant, understand that participation in programs, activities, sports, camps, field trips, and events organized by Be Well Kids Foundation may involve physical activity and certain risks of injury. I voluntarily allow my child to participate in these activities and assume all risks associated with participation, including but not limited to accidents, falls, injuries, illness, transportation incidents, or property damage. I hereby release, waive, discharge, and hold harmless Be Well Kids Foundation, its directors, officers, volunteers, staff members, sponsors, partners, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may occur during participation. I certify that my child is physically able to participate in the activities and that I will disclose any medical conditions, allergies, or special needs that staff should be aware of. I authorize emergency medical treatment for my child if necessary and understand that I am responsible for any medical expenses incurred.
